Dietetic Internship

1. OFFICIAL TRANSCRIPTS

Official transcripts from all colleges and universities attended should be sent to DICAS- Transcript Dept., PO Box 9118, Watertown, MA 02472.

2. DICAS APPLICATION

Didactic program in dietetics verification statement. If the student has not yet completed, then an "Intent to complete" form signed by their DPD program director (from an ACEND-accredited university/college).

3. REFERENCES

When completing the application form, applicants must include the name and contact information (specifically an e-mail address) for each reference. This will trigger an e-mail message requesting completion of a reference form.

4. PERSONAL STATEMENT

Questions to be addressed in the personal statement (1000 word limit):
Why do you want to enter the dietetics profession?
Discuss experiences that have helped to prepare you for your career.
What are your short-term and long-term goals?
What are your strengths and weaknesses or areas needing improvement?

In addition to the DICAS requirements outlined above, you also are required to mail an application fee of $40 by February 1st as a check or money order to Loyola University Chicago. Please mail to:
