Censure Complaint Form

Please complete all sections of this form to submit a complaint to Attorney General Gutierrez to file with the Judicial Board. If you have any questions, please email pgutierrez3@luc.edu. Note that you may not file complaints anonymously and that anyone in the Loyola Community may file a complaint.

If you perceive a conflict of interest, you may consult the President, Chief Justice, Speaker, or Staff Advisor for an alternative means to have your complaint reviewed. Please allocate at least 24 hours for the Attorney General's response to your complaint, and expect a follow-up meeting or email exchange concerning your completed form.

FOIA Form

The Freedom of Information Act (FOIA) has provided the public the right to request access to records from any federal agency. Federal agencies are required to disclose any information requested under FOIA unless it falls under one of nine exemptions which protect interests such as personal privacy, national security, and law enforcement. FOIA also requires agencies to proactively post online certain categories of information, including frequently requested records.
